A missing Long Island man who police thought to be in need of medical attention has been located, authorities said. 

Charles Ford, age 58, had last been seen on Thursday, Sept. 21 at 9 a.m. leaving his West Hempstead home. 

On Monday, Sept. 25 -- four days after he first went missing -- Nassau County Police announced that the man had been found. 

Additional details, including where he was found and his current condition, were not released.

A man with dementia who may be in need of medical attention has disappeared after leaving his Long Island home, according to authorities searching for him.

Charles Ford was last seen at 9 a.m. on Thursday, Sept. 21 leaving his home in West Hempstead on Hempstead Avenue, according to Nassau County Police.

The 58-year-old has dementia, authorities said, and he may be in need of medical attention. 

When last seen, Ford was wearing a red shirt, a gray hoodie, black pants, yellow socks, and white flip-flops.

He is described as 5-foot-11 with brown eyes and gray hair; he weighs approximately 160 pounds.

Police reported that Ford’s destination may be South Hempstead.
